424 High Volume vs. Low Volume Transversus Abdominis Plane (TAP) Blocks and Post-Operative Pain in Elective Colorectal Surgery- the More the Merrier?

Abstract Introduction Transversus abdominis plane (TAP) regional anaesthetic blocks are often performed by surgeons and provide effective anaglesia in the immediate postoperative period. It is not known whether volume concentration of local used for TAP influences quality analgesia when total drug dose similar. Aim This pilot aimed to compare low versus high-volume block post-operative pain patients undergoing elective colorectal surgery, using analgesic consumption as a proxy pain. Method Single centre cohort study. Data were collected resections from April – December 2021. Patients cohorted into those receiving low-volume, high-concentration (40–50ml 0.25% levobupivacaine) high-volume, low-concentration (100ml 0.125% levobupivacaine). including PCA (patient-controlled analgesia) type (morphine/ oxycodone) collected. Results 107 identified. regarding usage was available 55 patients. 23 (41.8%) given high blocks. 78.3% (p=0.56) group prescribed postoperatively, compared 84.3% low-volume group. Median morphine 11mg cohort, 18mg (p=0.23). Conclusions High associated with reduced requirement PCA.
